Organizational Accidents Revisited

Reason presents a follow up to his 1997 book Managing the Risks of Organizational Accidents. This text is focused on expanding the ideas in the first book by examining and analyzing the causes of ten organizational accidents that have occurred over the past twenty years. The book also covers the anatomy of an organizational accident, and error-enforcing conditions. The author is a retired faculty member of the University of Manchester in the United Kingdom.
