Goodman, an economist and specialist in health policy, retirement reform, and tax issues, examines the problems with the Affordable Care Act (ACA) and solutions that will empower patients and
competition among insurers and providers and minimize the role of government. He outlines the problems--its inflexible and weakly enforced mandate, its capped subsidies, the lack of medical
personnel to meet patient demand, the effects on the economy and employers, and incentives that encourage insurers to overcharge the healthy and undercharge the sick--and proposes six
principles for an alternative: choice, fairness, universal coverage, portability, patient power, and a real insurance market in which the insurer would receive a premium that reflects the
enrollee’s expected actuarial cost. In the third section, he presents recommendations for better affordability, access, and quality: replacing the mandates and subsidies with a universal tax
credit that is the same for everyone, replacing the different types of medical savings accounts with a Roth Health Savings Account, allowing Medicaid to compete with private insurance, and
denationalizing and deregulating the exchanges and requiring them to create change-of-health-status insurance. The book is based on his other book Priceless: Curing the Healthcare Crisis and
